Documents needed to lodge a Claim:
Death Claim
- Original copy of the policy document
- Death certificate
- Cause of Death Report
- ID of the Policyholder and the beneficiary(ies)
- For mortgage claims, we require an official letter from the bank and loan statement
- Documents are submitted to HSI Claims Committee for evaluation and approval
In case of death for individual life policies, the beneficiary (ies) or next of kin should notify the Life Department within 24hrs, providing all supporting documents and payment will be made within 48hrs.
Total Permanent Disability Claim
- Original copy of the policy document
- Medical report from the Medical Board confirming cause of TPD
- Policyholder’s ID
- For mortgage claims, we require an official letter from the bank and loan statement
- Documents are submitted to HSI Claims Committee for evaluation and approval
